You need to have an anvil* in your inventory, wield a hammer and find a forge. Then you'll need something to improve - weapons or armour - and lots of ingots (which can be got from ore). To smith, you stand on the forge and use the smithing skill. Your chances of successfully improving an item depend on how high your smithing skill is, and there are certain limits as to how far any given item can be improved. Smithing takes a lot of time and you need some amount of patience, but it is actually quite powerful. Don't even bother to try improving an item before your smithing skill is at 100, though. Until then, you'd better train the skill by melting ore into ingots.
*anvils are fairly rare, but an easy way of getting both an anvil and access to a forge is to teleport Glod away from his forge, lure him down the stairs (might be tricky) and kill him.
